Position Summary

The Social Worker provides case management and goal-oriented care planning for women accessing supports through the Women’s Emergency Accommodation Centre (WEAC). Working with people who identify as women, aged 18 and older, who are experiencing homelessness or housing instability, the role uses a trauma-informed, Indigenous worldview and person-centered approach to assess needs, reduce barriers to wellness, and promote stability. As part of a multidisciplinary team, the Social Worker collaborates with staff and provides guidance, education, and support to Case Workers to help women achieve their self-identified goals.

Schedule

* Monday to Friday, 8:00AM – 4:00PM
* Every five weeks, employees transition to a rotating schedule that includes:
* Monday to Wednesday: 3:00PM – 11:00PM
* Saturday and Sunday: 10:00AM – 8:00PM
* Thursday and Friday: Days off
* Participation in an on-call rotation is required 

Key Responsibilities

* Develop and implement individualized, person-centered, goal-oriented care plans that promote stabilization and pathways to permanent housing. 
* Provide comprehensive case management, advocacy, and system navigation to support women in achieving their self-identified goals. 
* Collaborate with multidisciplinary teams, clinical staff, and community partners to coordinate services and connect women with long-term supports. 
* Support and mentor Case Workers in the implementation of goal-oriented care plans and relationship-based, harm reduction practices. 
* Foster meaningful relationships with women, strengthen natural supports, and promote successful transitions to permanent housing. 
* Maintain accurate, timely, and confidential documentation, reporting, and participant records to support quality service delivery. 
* Develop and maintain community resource information while promoting collaboration, professional practice, and continuous improvement across the program.

Qualifications

Education

* Diploma or Degree in Social Work
* Registered with the Alberta College Social Workers (ACSW)

Certification

* Standard First Aid and CPR
* Suicide Intervention
* Trauma Informed Care
* Harm Reduction
* Non-violent Crisis Intervention

Experience

* 1+ year of experience conducting case management and developing individualized care plans. 
* 2+ years of experience supporting marginalized and vulnerable populations with complex needs. 
* Strong knowledge and application of relationship-based, harm reduction, person-centered, trauma-informed, strengths-based, collaborative, and solution-focused practices. 
* Experience supporting individuals experiencing trauma, mental illness, addictions, poverty, and other complex barriers using evidence-based models of practice. 
* Proficiency with Microsoft Office applications and database systems for documentation and case management.

Compensation and Benefits

* Competitive Salary: $51,578.80 – $62,489.70 (based on experience)
