Job Description
Position Summary
On a flexible, as-needed basis, provides clinical services to all residents residing at Communities of Christian Living Communities (CLC). These services will include skilled nursing, assisted living, and/or rehabilitation services.
Rehabilitation Flex Team Members are exempt from a multiple-shift requirement, and are encouraged to cover needs at multiple sites as they are able.
Assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and support recovery from injury or illness. This role goes beyond clinical treatment – it’s about building genuine, trust-based relationships with residents and their families, and contributing to a team culture that values the unique strengths each person brings, both on our staff and among the residents we serve.
Essential Duties
Restores residents’ function, alleviates pain, and prevents further limitations by planning and administering medically prescribed physical therapy.
Provides quality, personalized care by assessing and interpreting evaluations and test results.
Develops a Plan of Treatment for each resident based on initial and ongoing evaluations, including short- and long-term goals, collaboration with other staff members, and appropriate resident and caregiver involvement.
Determines physical therapy treatment plans in consultation with physicians or by prescription.
Implements the Plan of Treatment on a consistent schedule, adjusting the plan as needed based on resident progress.
Takes time to understand each resident’s goals, preferences, and comfort level, fostering trust throughout the treatment process.
Helps residents accomplish their treatment plan by administering manual exercises, providing massage therapy, and using equipment as necessary.
Evaluates mobility, strength, and current level of function, setting goals that are scaled to each resident’s abilities and realistically attainable.
Evaluates and records resident progress, modifying treatment plans and introducing new approaches as needed.
Evaluates the use and fit of prosthetic and orthotic devices, observing resident progress and recommending adjustments and modifications.
Documents resident care services according to approved medical and facility documentation procedures.
Communicates resident progress with warmth and clarity to residents, caregivers and loved ones, facility staff, and other healthcare professionals involved in ongoing care.
Develops a transition plan for residents following completion of their Plan of Treatment, coordinating appropriate in-home care services.
Collaborates closely with fellow team members, contributing to a supportive, inclusive workplace culture where diverse backgrounds and perspectives are welcomed and celebrated.
Qualifications
Basic Qualifications & Experience
Education: Graduate of an accredited therapy program.
Experience: 2–3 years of related experience, or an equivalent combination of education and experience.
Licenses/Certifications: Current Physical Therapist state license; CPR certification.

Working Conditions / Physical Requirements
Physical stamina to lift, transfer, and assist residents through physical activities.
Requires frequent lifting and carrying of items weighing up to 50 lbs. unassisted, including assisting residents when needed.
Must be able to communicate clearly with residents in person and over the phone.
Visual acuity adequate to perform job duties, including visual examination of residents and reading information from printed sources and computer screens.
